Facility Use Form
The Facility Use Form is available here. To make reservations for the covered picnic area at Panzak Park you must first call the City of Fowler at (559) 834-3113 ext. 100. Information on availability and any applicable fees will be provided. The completed reservation form and fees must be submitted to the City of Fowler before reservation will be confirmed.
